Before Your First Visit

Did You Know?

California Direct Access Law allows you to see a Physical Therapist right away. This means you can get evaluated and treated for any issues involving pain and movement without having to see a doctor before getting treatment.

By phone or by email, we briefly talk about your problem and determine the right type of service for you, the appropriate location for appointments, discuss payment and insurance, and I'll answer any other questions you may have for me.

If it seems like a good fit, then we'll schedule an appointment. An email confirmation with intake paperwork will be sent out within 1 day of scheduling. You can fill out the forms before your appointment so we can spend as much time as possible focusing on your needs in the first session.

Reminders for your appointment are sent via text or email 24 hours prior to the appointment time.

Covid 19 Notice

As we continue to navigate the COVID-19 pandemic and its aftermath, there is nothing more important to me than your health and safety.

Masking is required for all patients from November 1 to March 31 per the Santa Clara County Public Health Department. Outside of the stated time frame, masking is optional.

What am I doing to keep you safe?

  • I am up to date on all vaccinations and boosters
  • I wear a high-quality mask (N95) at all times during session
  • I wash my hands and/or hand sanitize before and after contact with each person or else I use gloves during sessions
  • I use a HEPA air purifier for ventilation in the office
  • All equipment is cleaned and disinfected after each use

What can you do to keep me safe?

Cancel or reschedule your appointment if you are feeling sick or have been exposed to a person who tested positive for Covid-19 and have not been tested yet.

While masking is not required in healthcare settings outside of the November to March timeframe, the California Department of Public Health still recommends masking if you are experiencing any respiratory symptoms, such as a cough, sore throat, or runny nose.

IMPORTANT

If you are symptomatic with Covid 19, you must wait 10 days since onset of symptoms, have no fever without use of medication, and improving symptoms prior to your next session.

If you have recently tested positive for Covid 19 but have no symptoms, the California Department of Public Health recommends wearing a well fitted mask (N95, KN95, or KF94 preferred) for 10 days after receiving your positive result. We will reschedule your appointment for 10 days after your positive test or once you have a negative Covid test.

We will cancel and reschedule your appointment. The late cancellation fee will be waived.

Get Ready for a Session

Your time and your health are precious. To make the most out of our sessions, you can take these steps to prepare for them no matter where we meet.

  • Wear loose comfortable clothing that provides easy access to your body and you can move around in. (Activewear is a great option)
  • Reschedule if you are feeling sick or have tested positive for Covid
  • Be open to learning and curious- ask questions!

Meeting in your home? I will bring basic exercise and therapy equipment as well as a massage table to sessions. However, there are some of the things you can do to help make the most of out of our sessions.

  • Clear a 9'x5' space if we are meeting in your home
  • Make sure your pets don't try to "help" or "get their own treatment" during our sessions
  • Have your own workout equipment available for us to use

Understanding your medical history is important, so gather together (if available):

  • Physical Therapy prescription
  • Imaging (X-rays, MRIs, CT scans, etc.) reports or DVDs
  • Surgical reports
  • Medication List
  • Lab reports
  • Any other documents you think might be helpful for me to see, including your medical history and other PT exercises
